This is certainly true of transperineal biopsies. Although transperineal biopsies are better in many aspects from the transrectal ultrasound or TRUS biopsies, for them to offer you your best short-term and long-term outcomes including the mitigation of potentially life altering side-effects from future treatment, you need to perform the transperineal prostate biopsy with a specific type of ultrasound not commonly found in most medical offices or clinics, the 3D Color-Flow Power Doppler Ultrasound.

It’s advantages over the traditional, and more commonly used, 2D greyscale ultrasound are numerous, and make all the difference in your outcomes both in terms of diagnosis via the biopsy, and later with any eventual prostate cancer treatment.

When it comes to prostate biopsies, many consider Transperineal Prostate Biopsies to be far superior to the Transrectal Prostate Biopsies. Unfortunately, most insurance companies will decline coverage without first having a series of negative prostate biopsies in combination with other positive prostate cancer factors such as progressively climbing PSA values indicating the presence of something brewing. However, there are many advantages to utilizing Transperineal Prostate Biopsies right from the start.

Transperineal Ultrasound Prostate Biopsies

  • Transperineal Prostate Biopsies avoid the rectum altogether mitigating additional risk of infection due to the biopsy needle passing through the rectum
  • Transperineal Prostate Biopsies offer a more precise way to sample the prostate due to it’s ability to access various regions more effectively than Transrectal Prostate Biopsies
  • Transperineal Prostate Biopsies avoids rectal bleeding and significantly reduces the potential for sepsis
  • Transperineal Prostate Biopsies have been associated with a more comfortable prostate biopsy experience as they are done under anesthesia greatly reducing discomfort

Clearly Transperineal Prostate Biopsies are the preferred prostate biopsy, especially when the patient’s needs are considered. Make sure you discuss the advantages and disadvantages of both Transrectal Prostate Biopsies and Transperineal Prostate Biopsies with your Urologist. Further, make sure you do your own research with regards to both common types of prostate biopsies so you are armed with the necessary information to be a great patient advocate for yourself.

3D Color-Flow Power Doppler Ultrasound Guided Prostate Biopsies offer superior diagnostics and Outcomes

Unlike mainstream methods for MRI guided 2D greyscale transperineal prostate biopsies, 3D Color-Flow Power Doppler Ultrasound Guided Prostate Biopsies offer superior diagnostics and Outcomes

  • With 3DCFPDU prostate biopsies your prostate is mapped in 3 dimensions as opposed to 2 dimensions giving you a more precise image, which leads to more precise targeting of suspicious areas.
  • You can identify areas of increased vascularity. Prostate cancer, like other tumors, display increased angiogenesis, resulting in higher micro-vessel density. Often, this vascularity is ever so nuanced and slight, and can only be seen with 3D Color-Flow Power Doppler Ultrasound. You simply can’t see this level of detail with a 2D greyscale ultrasound as it doesn’t have the capability to display vascularity, period. Color Flow offers visualization of blood vessels within the prostate, a key indicator of the presence of cancer.
  • 3DCFPDU guided prostate biopsies substantially increases the correct targeting of malignant tumors by visualizing hypervascularity. Standard prostate biopsies simply can’t provide this level of detail, and this alone could be the difference between successful prostate cancer treatment and further on recurrence and complications.
  • 3D Color Doppler Ultrasound for prostate biopsies allow for more selective prostate biopsies giving you more precise data points so you can rest assured you will have biopsied as many points as possible that are suspicious. Points you may miss by using an MRI Guided 2D greyscale prostate biopsy procedure. Remember a higher number of highly selective biopsy sites ensures your best outcome overall. This is especially true when you wind up with a negative biopsy. It’s possible, and perhaps even likely, that using biopsy targeting with 2D greyscale ultrasound imaging, that you could miss spots that would test positive. Using vascularity as a targeting mechanism via 3D Color Flow Ultrasound ensures your highest chances that no potential cancer location is overlooked. Dattoli pioneered prostate biopsies utilizing 3DCFPDU gives you your best chance to avoid future complications from false negatives. It allows you to catch even the most nuanced prostate cancers that standard prostate biopsies won’t. It’s better to spot them early when they are easily treatable with minimally invasive prostate cancer treatment.
  • Prostate biopsies performed with 3DCFPDU are also beneficial when it comes to those patients that are put on observance instead of treatment because the scoring isn’t high enough to warrant treatment by current day standards. It’s possible that when a more accurate prostate biopsy such as a 3DCFPDU guided biopsy, the overall numbers tell a different story and would warrant further investigation and perhaps treatment. Putting off treatment because of a less precise prostate biopsy can lead to more in depth treatment later in life, and again, potentially more life altering side effects. Utilizing more precise prostate biopsies in your diagnosis is the best way to address any cancer now, or later. Make sure you see as much as you can and advocate for yourself, demand 3DCFPDU guided prostate biopsies.
  • Prostate biopsies when performed with an MRI and 3DCFPDU in combination offers you the most precise prostate biopsy available anywhere, and improves greatly, cancer detection rates as it allows for trusted, comprehensive sampling from various prostate zones, some of which can’t be accessed with 2D greyscale ultrasound or the standard transrectal procedure for prostate biopsies.
  • For patients that can’t tolerate MRI’s with contrast because of kidney function or other conditions, 3DCFPDU offers an alternative with high precision.
  • mpMRI guided prostate biopsies are only a snapshot of a point in time which doesn’t offer the patient true real time information and mapping such that you realize when using 3D Color-Flow Power Doppler Ultrasound during the procedure. In real-time you can sample and investigate suspicious areas of the prostate the original MRI didn’t pick up. Being able to sample suspicious previously determined negative areas from the MRI is critical in precise data gathering and successful cancer determination and treatment if necessary.

Prostate Biopsies – MRI and 3D Color-Flow Power Doppler Ultrasound Guided Transperineal Prostate Biopsies

Over the past few decades, significant advancements in transperineal prostate biopsies via technology improvements in prostate cancer imaging have revolutionized the detection and diagnosis of this disease. While multiparametric magnetic resonance imaging (mpMRI) has greatly improved the detection of prostate cancer, it does however come with certain drawbacks. The readings of mpMRI can differ significantly due to challenges in interpretation, lack of standardization, and the staffing challenges inherent when a high level of expertise is required to interpret mpMRI images accurately.

Further, for some patients, the requirement of contrast during the prostate biopsy imaging can cause kidney issues and trigger allergic reactions. additionally, mpMRI’s are merely a snapshot in time and don’t provide the real-time imaging you need to obtain the most precise prostate biopsy possible.

3D Color Flow Doppler ultrasound (3DCFD) has the benefit of enhancing the traditional 2D greyscale ultrasound by providing color-coded blood flow information, which can reveal the hypervascularity often associated with cancerous lesions in the prostate. Cancers are hungry for nutrients and secrete growth factors to create new blood vessels to feed it so it can grow larger, faster and spread(1). Sometimes cancers can be too small to be seen or mimic normal tissue and appear as normal tissue. The appearance of abnormal blood vessels can suggest the presence of cancer. 3DCFD has the added advantage of being able to visualize the prostate in real time while the prostate biopsy is taking place.

However, no one imaging modality is 100% accurate and can detect all cancers. With all tests, the accuracy depends on the sensitivity as well as the specificity of the test. Sensitivity refers to the test’s ability to correctly identify those with the disease (true positive rate), whereas specificity refers to the test’s ability to correctly identify those without the disease (true negative rate). A highly sensitive test means that there are few false negatives, which is crucial for early detection of diseases. Conversely, a highly specific test means that there are few false positives, which is important to avoid unnecessary treatments for those not afflicted by the disease. The ideal diagnostic test would have both high sensitivity and high specificity, but in practice, there is often a trade-off between the two.

By combining multiple tests, we can achieve higher accuracy in detecting cancer by targeting prostate biopsies more precisely. Newer ultrasound techniques have been compared to mpMRI. Data suggests that the sensitivity of ultrasound for clinically significant prostate cancer is comparable or superior to mpMRI, however, specificity is comparable or mildly reduced(2). By combining the two modalities, targeting can be improved.
